What is Breast Augmentation?

Some of the most common questions asked of the Nashville Plastic Surgery Institute team are: “What is done in breast augmentation?” and “Who is breast augmentation for?”

Breast augmentation, a type of surgical breast enhancement—also referred to as a “breast aug” or a “boob job”—is a common procedure that can give you the body you’ve been dreaming of. For many patients, breast augmentation can provide a needed boost to self-confidence.

Breast augmentation creates a rounder breast shape and restores depleted volume. The procedure has been around for many decades, with hundreds of thousands of satisfied patients choosing it from its early days of conception to 2022 and beyond. Thanks to constant advances in medical technology and surgical techniques, breast augmentation is a versatile procedure that can help patients achieve many different aesthetic goals.

Who Makes an Ideal Breast Augmentation Candidate?

“Am I a good candidate for breast augmentation?” The best way to find out for certain is to book a consultation with a board-certified plastic surgeon. Patients who decide to have breast augmentation may include those who have always been dissatisfied with the natural size, shape, or projection of their breasts, those who simply want to add more fullness, and women who wish to restore their breasts to their previous shape after pregnancy, weight fluctuations, or aging have caused a loss of volume and a change in shape.

Ideal patients for a breast augmentation may include those who:

  • Are you in good physical and mental health
  • Are not currently breastfeeding or pregnant
  • Have breasts that are fully developed
  • Have one breast that is a different size or shape from the other
  • Feel that their breasts are too small
  • Want to add more fullness to the upper portions of the breasts
  • Are bothered by the shape, firmness, or projection of their breasts
  • Have one or both breasts that did not develop normally
  • Maintain realistic expectations and an optimistic outlook

If you are currently pregnant, breastfeeding, have recently stopped breastfeeding, have a severe illness or infection, have untreated breast cancer, or have an abnormal mammogram or undiagnosed breast condition, breast augmentation may not be right for you.

Breast Augmentation Can:

  • Increase the fullness and projection of your breasts
  • Improve the proportion of your figure
  • Correct imbalanced or asymmetrical breasts
  • Enhance your self-image and self-confidence

What Does Breast Augmentation Surgery Involve?

There are several decisions patients will need to make with their surgeon for breast augmentation, including implant type or filling, implant shape, implant profile, and other surgical details such as what incision type should be used, along with which placement type and technique is best.

Types of Breast Implants

Breast augmentation involves the use of breast implants. Implants are prosthetic inserts that are used to add volume to the breasts and alter their shape. Implants come in two major categories: saline and silicone. There are pros and cons to each type. Traditionally, saline implants have been more popular, but that is changing with the newest generation of silicone implants used for breast augmentation.

Saline breast implants are made of a silicone shell and are empty when inserted into the breast pocket. After insertion, these implants are filled with a sterile saltwater solution called saline.

The surgical technique used for saline implants is different than the technique used for silicone implants because saline implants don’t require as large of an incision. Saline implants can be inserted through shorter and smaller incisions. Due to this, saline implants may be the only type available for certain implantation techniques, such as the transumbilical surgical technique. Saline implants come in a variety of sizes and are available for patients who are 18 and older.

Silicone implants are made of a silicone shell and are pre-filled with silicone gel. Newer generations of silicone implants are often referred to as “gummy bear” implants because of the solid-state gel inside of them. Gummy bear implants are said to have a more natural texture and also pose less of a risk of leakage since they hold their shape if punctured.

Silicone implants are inserted pre-filled and typically require a larger incision than saline implants. These implants are available to patients who are at least 21 years of age.

Breast augmentation at our Nashville, TN, practice can involve one of four incision locations: periareolar, transaxillary, transumbilical, or inframammary.

There are four main areas at which incisions may be placed for breast implant insertion: bordering the areola or the colored area around the nipple (periareolar); within the breast crease (inframammary); within the armpit or axilla (transaxillary), or at the navel (transumbilical). The best option for insertion differs for each patient, depending on their physical characteristics, goals, and type of implant to be used.

Breast Implant Shape

An important factor that needs to be considered for breast augmentation is the shape of the implants. There have historically been two shapes available: round and teardrop (also known as “anatomical”). Round implants are the most common option and are ideal for patients who want to add lift, cleavage, and volume because they have a full, “circular” shape that creates more volume at the tops of the breasts. Teardrop-shaped implants have bases that are fuller than the tops, giving them a tapering appearance that looks very natural—but these shapes are not often used anymore. A new technology that fills round gummy-bear implants with various levels of gel firmness can create a teardrop shape without the need for an actual anatomically shaped shell. The most suitable shape for you depends on your body type and goals.

Breast Implant Profile and Projection

Breast implant projection refers to how far the breasts project outward from the chest wall when the patient is standing. There are several different categories, including low profile, moderate profile, and high profile. The right profile for each breast augmentation patient depends on their body proportions and desired results.

Breast Implant Placement Options

Another aspect of breast augmentation is the surgery involved to place the implant in the breast pocket. Depending on the concerns of the patient and the patient’s body type, there are a number of techniques that differ based on where the implant is inserted. The best option for you is determined during a consultation with Dr. Unger.

Implants can be placed in a sub-glandular, subfascial, or submuscular pocket. As of 2022, the majority of breast augmentation patients have their implants placed beneath the pectoral muscle in the upper portion of the breast.

The implant pocket affects the appearance of the implant and the tissue coverage around the implant. This is determined based on a number of factors, including the amount of supportive tissue the patient may have in that area of the body.

Planning Your Breast Augmentation Procedure

At NPSI in Nashville, breast augmentation procedures are performed by Dr. Unger. As a board-certified plastic surgeon he aims to provide the most proportional, natural-looking results possible. Individualizing every patient with respect to their existing breast tissues, their desires, and appropriate implant choices is the keystone of a successful breast augmentation outcome.

Dr. G. Patrick Maxwell developed the concept of “bio dimensional planning” to measure his patients’ existing breast tissue very precisely—a step that Dr. Unger will take for you in planning your breast augmentation. This allows for high-quality results that are personalized to a patient’s existing shape and better match her desires.

In order to allow you to “see” (or predict) how your own body will look with different shaped and sized implants, Dr. Maxwell invented an advanced 4-D imaging system that will be used in your consultation. Based on this individualized interaction and taking into account your desires and concerns, your surgeon will share their surgical vision and discuss your breast augmentation options before creating a surgical plan customized to match your body and goals.

They will discuss various breast augmentation options with you, including:

  • Implant type: Each patient is unique, so Dr. Unger will personally help you custom-select the size and shape of silicone gel or saline implant that offers the best results for you.
  • Placement: Dr. Unger will discuss your breast augmentation implant placement options, either above or below the pectoralis muscle, depending on your tissue quality, breast volume, and preference.
  • Incision: Dr. Unger typically will insert your implants through an incision hidden in the breast crease or at the edge of the areola (the darker skin around the nipple), again depending on your individual needs.

Dr. Unger understands that many women are concerned with the possibility of scarring, so they utilize techniques that ensure a smooth, highly natural-looking result while minimizing the appearance of any scars. He also offers approaches specifically designed for breast reconstruction and breast implant revision patients.
